Group Home Shift Leader - Boone, NC 

Assure residents’ health and safety while providing a home atmosphere.  Administer medications and support residents in their home with personal care, medical needs, meals, and daily living skills.  Implement participants’ goals according to their individual plans.  Supervise staff on shift and assure direct care is provided.  Provide positive community integration experiences including shopping, socialization, and money management.  Keep accurate records and documentation of daily activities and progress for each participant/resident as required by applicable standards.  High school education and two years of experience in related area or equivalent combination of education and experience required.  Need ability to effectively work with adults with developmental disabilities.  Background check, MVR check, Driver's License with acceptable driving record, and reliable vehicle required.  Work 7 consecutive days (Wednesday to Wednesday) that include staying overnight with time off during the day on weekdays, followed by 7 days off.  Full Time with excellent benefits including Medical, Dental & Life Insurance, Vacation/Holiday/Personal paid time off, Matching Retirement, Travel Reimbursement, etc. 

Community Employment Job Coach - Boone, NC

Support individuals with disabilities in obtaining a job, learning workplace expectations, and performing job duties in a community employment job.  Develop community employment sites and provide on-site training of job responsibilities and appropriate social skills for adults with disabilities.  Assess individual interests, natural supports, and skills to assure job placements are compatible with each individual’s needs.  Maintain on-going communication and positive relationships between employers, program participants, and various agencies.  Need ability to work with adults with disabilities.  Must be flexible in work duties and work hours (will include some nights, weekends and holidays).  High School Diploma and 4 years of work experience required or equivalent combination of education and experience.  Position requires regional travel serving Ashe, Avery, Watauga, and Wilkes counties.  Office located in Boone.  Background check, MVR check, Driver's License with acceptable driving record, and reliable vehicle required.  Full Time with excellent benefits including Medical, Dental, Vacation/Holiday/Personal paid time off, matching Retirement, Travel Reimbursement, etc.

Assistant Director of I/DD Services - Boone, NC

Coordinate and provide community opportunities for adults with I/DD.  Plan and implement regular training activities including social, health, safety, transportation, recreation, vocational, and volunteer programs.  Assure scheduling of direct support instructors that meets participants’ goals and needs.  Supervise staff providing services and ensure timely completion of all documentation in EHR system.  Responsible for program implementation, documentation, and completion of billing/eligibility criteria in accordance with funding and accreditation standards. Ensure implementation of individualized support plans.  Maintain effective working relationships with service participants, families/caregivers, staff, funding sources, community agencies, and others as appropriate.  Bachelor’s degree in human services or related program and two years of experience providing services to adults with intellectual developmental disabilities, or an equivalent combination of education and experience required.  QP status required.  Must have experience with I/DD Medicaid funded services and be able to work in a team environment, supervise and train staff, and coordinate service delivery and plan meetings.  Need ability to exchange information and to communicate progress and needs of participants in a variety of settings.  Must be able to actively assist participants in varied activities and locations related to service delivery.  Computer and EHR proficiency required.  Background check, MVR check with acceptable driving record, and reliable vehicle required.  Full-Time, Exempt.  Monday-Friday, daytime schedule.  Excellent benefits including Medical, Dental, Vacation/Holiday/Personal paid time off, matching Retirement, Travel Reimbursement, Life Insurance, Hiring/Referral bonus, etc.

Part-Time Positions

Group Home Personal Care Attendant - Boone, NC

Assist residents in their home with personal care and hygiene goals, daily living skills, personal safety, recreation/leisure skills, and community activities, i.e., shopping, eating out, exercise, recreation, church, etc.  Implement residents’ goals according to their individual plans.  Administer medications and prepare meals.   Keep accurate records and documentation of daily activities and progress for each participant as required by service standards.  Act as an advocate and communicate professionally with other agencies, families, staff, etc.  High school diploma and ability to effectively work with adults with developmental disabilities required.  Need ability to keep accurate records and work independently.  Must be able to actively assist participants in varied activities and locations related to service delivery.  Background/MVR check with acceptable driving record, Driver’s License, and reliable vehicle required.  Part Time.  Evening and Weekend hours, 15-25 hours per week.    Benefits include paid time off and mileage reimbursement.
